6. DIOCESAN ATHLETIC FEE: There shall be a Diocesan Athletic Fee of $3.00 per child per sport for all children who participate in approved Diocesan Interscholastic Sports Programs. These fees should be collected by each individual school and submitted to the DAD in the form of a check along with the Team Roster.
7. DIOCESAN ATHLETIC INTERSCHOLASTIC REGISTRATION FORMS: Parents of athletes must complete the Diocesan Athletic Interscholastic Registration Form prior to practice. The Registration Form MUST be signed by a physician. This form includes:
I. Physicians Certificate valid if dated after June 1 and good for the entire school year.
II. General Information.
III. Name of Medical Insurance Company and policy information. Each athlete must be covered by medical insurance. The school will provide a secondary insurance policy for all uninsured athletes.
IV. Parent Consent Statement.
V. CCD eligibility information, if applicable.
This form need be completed only one time during the school year. Coaches must have a copy of this completed form for each athlete and should keep it on hand for all games and practices. The original of this form must be on file in the principal's office before he/she can participate in any form of the Diocesan Interscholastic Athletic Program. Coaches must receive from the principal a copy of the registration form previously submitted by the athlete.
8. SANCTIONS: Any school found to be guilty of allowing an ineligible student to participate in interscholastic athletic events shall immediately forfeit any wins, records or awards as a team for all athletic events in which the ineligible student participated. In addition, any individual win, record or award shall be forfeited by the ineligible student.
If a coach doubts the eligibility of any player on a team, he shall have both the right and the duty to ask for and receive proof of such eligibility from the Commissioner. The DAD, where appropriate, will make the decision concerning the eligibility of any player.
9. DIVIDING TEAMS EQUALLY: When used in these bylaws or the rules for a particular sport, the method to be utilized by a school to divide its teams equally shall be to divide the available talent pool into “A,” “B,” and “C,” rated players and each team will have approximately the same number of “A,” “B,” and “C” players at the conclusion of the selection process. The athletic director and/or PE teacher shall review the rosters to assure that the teams are divided equally. |
