10. STEPS FOR SUBMITTING DIOCESAN REGISTRATION FORM AND ROSTER FORM
1. Principals should meet with the schools' Athletic Directors to inform him/her of the Diocesan Guidelines for Interscholastic Sports.
2. Principals should require coaches to fill out a volunteer application/reference form and keep these on file in the office.
3. Coaches should have an organizational meeting with parents before the first practice. Diocesan registration forms should be explained to parents.
4. Principals should see that coaches do not allow players to participate in practice and/or games unless the registration form is filled out completely. (If a family should turn in a form without medical insurance, they should obtain a secondary insurance form from the school office.)
5. Once the registration form has been completed and turned in, the coach should make two copies of each form. He/She should keep a form for each player and carry to each practice or game. The second copy should be turned in to the school office.
6. As soon as the original team roster is completed by the coach, they should have three copies of this form made.
a. Coach receives one and keeps in his possession.
b. Coach gives the original to the principal. Principal should keep one in the school office with the registration forms.
c. Coach should submit third copy to the League Commissioner accompanied by the Diocesan Athletic fee.
If a coach wants to add a player(s) after the roster has been turned in, the principal must complete another roster form for that player(s) and submit it to the DAD or League Commissioner prior to their participation in practice or games.
Only players whose names appear on the roster form and submitted to the DAD with proper signatures may participate in the Diocesan Interscholastic League. |
